Submit your request

Use the secure form above to submit your request for Vitality GO. Once your request has been processed, you will receive confirmation that Vitality GO has been added to your policy and information on how to get started – including registering for your Vitality account and completing the Vitality Health Review.

With the Vitality member website and mobile app, you’ll get important tips and information about nutrition, exercise and more. You can easily record your everyday healthy activities like working out at the gym, walking the dog or buying healthy food to earn Vitality Points and rewards – and improve your health! The more active you are, the more points you’ll earn and the greater your Vitality Status and rewards.
